If you’ve scheduled or are considering your first SPRAVATO® treatment session, it’s completely normal to feel nervous. Most people aren’t afraid of the medication itself— they’re afraid of the unknown: What will it feel like? Will I be “out of it”? What if I panic? How long will I be there?

Step-By-Step: What Happens During a First SPRAVATO® Session

Your first appointment is typically the most “structured,” because the team is establishing a baseline and making sure you know what to expect.

Step 1: Check-in for a brief medical screening

You’ll check in, review any necessary consent paperwork, and the clinical team may ask about how you’re feeling that day. You’ll also have vital signs checked, especially blood pressure as esketamine can occasionally cause a temporary rise in blood pressure after dosing. 

If you have questions or concerns like, “Will this interact with my meds?” or “I’m anxious about dissociation,” this is a good moment to say it out loud. We expect questions and nerves, especially on visit one.

Step 2: Get settled in the treatment room

You’ll be brought to a comfortable area with a couch or reclining chair where you can sit back and relax during the monitoring window. Many people find it helpful to choose a low-stimulation plan like listening to a playlist and keeping eyes closed. Some people prefer gentle conversation or meditative practices; others prefer quiet. At the end of monitoring, the team will check how you’re feeling and decide when you’re clinically stable to go. You’ll leave with your ride and plan for a lighter day afterward. Remember: no driving, heavy or hazardous activities until the next day after restful sleep. 

What it can feel like (and what’s normal)

People often want to know what SPRAVATO® “feels like,” and the honest answer is: it varies.

Some patients feel sleepy or “floaty.” Some feel emotionally neutral. Many experience dissociation—often described as feeling slightly detached from your body, or like time and space feel different. This is one reason monitoring exists, and it’s also why many clinics encourage calming input (music, eyes closed, low stimulation). The medication can also cause dizziness or lightheadedness for some people, which typically improves as the session progresses. 

If you’re worried about panic, tell your care team. Fear is very normal and a safety plan exists to help you get through your session. You don’t have to “white-knuckle” through the experience.
